Science and inventions have played an important role in our lives. From the printing press to iPhones and computers and from vaccinations to artificial limbs and pacemakers, science has enhanced our way of life. However, science has also failed us. The reasons for these failures range from simple greed and/or incompetence to unexpected consequences. We will discuss several examples of scientific failures including thalidomide, DDT, and the Flint, MI water crisis. We will attempt to answer three questions:
1. What happened?
2. Why did it happen?
3. What were the long-term consequences from these technological failures?
